Count every child you receive or are applying for child benefit for — the child supplement is assessed per child. Between 1 and 20.

The gross income of all household members together, per month, in whole euros without separators — for example 2100.

Housing benefit is counted in the child supplement's income test. Tick the box if your household receives Wohngeld.

For example: the other parent works in the Netherlands, or the family already receives child benefit from Poland. Tick the box if this applies to your family.

SEPA direct-debit mandate

I authorise Form Pilots (payee, creditor identifier DE98ZZZ09999999999) to collect the one-time SEPA direct debit for this order's fee of 129,00 €. The debit will only be taken once the benefit has been paid out or the decision has been finally rejected.

For example DE02 1203 0000 0000 2020 51. Spaces and upper or lower case do not matter.

The name the account is held under, exactly as your bank has it.

Price and payment

Fixed price: 129,00 €

Payment only becomes due after you have received your money from the authority.

The amount will be collected by SEPA direct debit from the account whose mandate you grant below — as soon as the benefit has been paid out or the decision has been finally rejected.
